Hands often age faster than people realize — they are almost always exposed, almost always in use, and almost never included in the skincare routine that protects the face. Decades of sun, constant washing, repetitive motion, and natural volume loss all show up on the backs of the hands, frequently before the face shows comparable change.

But "aging hands" is not one problem. It is usually several happening at once: volume loss, prominent veins and tendons, thinning crepey skin, sun damage and brown spots, and texture changes from declining collagen and support. What we evaluate and treat

  • Volume loss — the fat layer that keeps hands looking full and youthful thins with age, leaving a hollowed, skeletal appearance.
  • Prominent veins and tendons — usually the result of volume loss and thinning skin above them, not a vascular condition.
  • Thinning, crepey skin — declining collagen and elasticity make skin fine, wrinkled, and fragile in appearance.
  • Sun damage and brown spots — decades of UV exposure that never received the protection the face got.

How it works

Why hands often age faster than people realize. The backs of the hands face the world constantly: daily UV exposure that rarely receives sunscreen, repeated washing and product contact, mechanical stress from everyday use, and skin that is thinner with fewer oil glands than facial skin. Meanwhile, the diligent skincare routines that protect the face almost never extend to the hands. The result is cumulative damage that becomes suddenly noticeable — often in the form of veins, spots, or thinning skin that seems to appear out of nowhere but has been developing for years.

Why veins and tendons become more visible. The back of the hand carries very little subcutaneous fat, and what it has thins with age. As that cushioning layer shrinks and the skin above it loses collagen, structures that were always there — veins, tendons, joints — simply become visible. In most patients this is a volume and support problem, not a vein disease, which matters because it means the right treatment addresses the lost volume and thinning skin rather than the veins themselves.

Volume loss, skin-quality loss, and pigment are three different problems. Volume loss produces the deflated, bony look and makes veins and tendons stand out. Skin-quality loss produces crepiness, fine wrinkling, and laxity. Pigment produces brown spots and uneven tone from sun exposure. Most hands carry some combination of all three — but they are driven by different biology and respond to different treatments, which is the core idea behind matching tools to the actual problem.

Why one treatment may improve one aspect but not all of them. Volume restoration replaces lost fullness but does nothing for brown spots. Pigment treatment clears discoloration but cannot restore volume. Skin-quality treatments rebuild collagen and texture but cannot replace lost fat. This is why a single product or single procedure so often disappoints — and why an evaluation that separates your volume, skin, and pigment concerns comes first.

The typical course

The typical path: a hand evaluation examining volume, skin quality, and pigment to identify your dominant aging factors; a plan that matches treatment to those factors — one approach or a short sequence, in the right order; then reassessment of results as they develop. Combination is never assumed — many patients need only one or two approaches. Your evaluation determines what your hands actually need, and nothing is added to your plan that does not address a specific concern.

Maintaining your results

Results are sustained with the same logic they were built: daily sun protection on the backs of the hands (the single highest-impact habit for preventing further damage), hand-appropriate skincare, and periodic maintenance treatment where it makes sense — volume refresh, collagen-building sessions, or spot treatment as needed. What maintenance looks like for you depends on which treatments you had and how your hands respond.

How Better Off approaches hand rejuvenation

Depending on what your evaluation shows, your plan may draw on one or more of three treatment families — selected for the specific factors aging your hands, not applied by default:

Volume restoration. Where lost fullness, prominent veins, or a skeletal appearance is the main concern, filler placed in the backs of the hands can restore volume where appropriate — immediately softening the visibility of veins and tendons. In selected cases, biostimulatory and regenerative approaches that encourage your own collagen over time may be recommended instead of or alongside filler. Which approach fits depends on your anatomy, the degree of volume loss, and your goals.

Skin quality. Where thinning, crepey texture is the main concern, collagen-building approaches are the workhorse: microneedling, regenerative support such as PRP/PRF, PDRN where appropriate, and selected skin-supporting topicals and adjuncts. These rebuild the dermal structure that makes hand skin look resilient rather than fragile.

Pigment and sun damage. Where brown spots and uneven tone dominate, options include appropriate peels, pigment-directed treatment, laser where appropriate, and selected topical brightening strategies — chosen for your skin type and the depth of the pigment, because the wrong approach on sun-damaged hand skin can disappoint or backfire.

No patient receives every treatment listed here. The plan is built from your evaluation — sometimes one approach covers your main concern, sometimes a short sequence of two addresses the combination.

What to expect — matching the treatment to the problem

  • Volume correction can soften the appearance of visible veins and tendons — by restoring the cushioning layer they became visible through. It does not remove the veins themselves, and candidacy depends on your anatomy.
  • Pigment treatment addresses discoloration, not volume loss — clearer tone makes hands look younger, but if hollowness or prominent structures remain, that is a separate problem with a separate solution.
  • Microneedling and regenerative approaches address skin quality rather than replacing lost volume — they rebuild collagen, thickness, and resilience over a series, and work best when volume loss is mild or is addressed separately.
  • Combination treatment may produce a more complete result when several aging factors are present — and when it is recommended, the sequence and timing are planned deliberately rather than everything at once.
